refactor: add workspaces root and @apexfusion/vector-mcp-shared

Extracts pure tx helpers, shared types, endpoint config, and the Ogmios
provider. Subpath exports (./tx vs ./provider) let the forthcoming signer
import pure helpers while being structurally unable to reach the network layer.
