Elrond 9a38b0510c fix(relay): report partial startup failures as degraded ...

Address review on the startup-step timing:
- NIP-43 and large-roster reconciles return ReconcileSummary
  {repaired, failed}; a sweep with any per-item failure ends its startup
  step degraded and the final log carries the failed count. NIP-FI warm
  degrades when any issuer did not warm.
- Startup step log field renamed outcome -> status to match lifecycle
  records.
- Log-capture tests pin exactly one finished record per terminal path
  (finish, degrade+finish, drop, panic) with its status.
- Progress predicate takes the reconcile purpose; table-tested so the
  maintenance sweep can never log progress.
- README: correct gauge idle-timeout description and note that a step
  outliving it loses its current series.
