Pull permissions into own server ( #50 )
...
This allows us to use the SDK version of an MCP server for all other
tool calls. Ideally the permission server goes away eventually.

Support plan mode and all other permission modes ( #40 )

Exposes all Claude Code permission modes as [ACP Session
modes](https://github.com/zed-industries/agent-client-protocol/pull/67 ).
This allows ACP clients to build mode selectors and to be notified when
the mode changes as a result of `ExitPlanMode`.

Use ACP terminal support when available ( #8 )

When the `terminal` client capability is available, we'll now replace
Claude Code's bash tools with our own that go over ACP. This includes
support for background terminals.
