fix: wire reconcile_provider_mcp_commands into app startup

Call reconcile_provider_mcp_commands in lib.rs between
reconcile_persona_pack_paths and migrate_persona_provider_to_runtime.
This ensures mcp_command values in managed-agents.json stay in sync
with the discovery table on every app launch.
